2/10/2023 EnchantaVaughn Approved B: QMHP arrived to the home of client. Upon arrival to the home setting, QMHP informed family of how QMHP will be his temporary counselor and that counselor would be providing at least 6-8 hours of services on a weekly basis. Mother and client was receptive to the information being provided to them and indicating to QMHP that she was just happy to have him the services in place with everything that is going on in the world today. Mother provided QMHP with some of the behaviors within the home and school setting such as anger issues fighting and talking back 2-3 times daily. Mother reports client being suspended from school with an 10 day panel for fighting in the school setting. Mother reports client just struggle with peer interactions. B: Client presented cooperative behaviors within the session as client was able to discuss with QMHP on why it is important for him to change his behavior. QMHP along with mother and client began to create with client his ISP goals and objectives. QMHP explained to mother as well as client the purpose of his goals and how he can work on achieving those goals as he is receiving services. I: QMHP utilized motivational interviewing approach involving active listening, clarifying questions, and the examination of consequences toward the client’s acceptance of responsibility and willingness to change anger control problems. QMHP commended client and mother for their willingness to process through the instances and communicate with each other afterwards. Likewise, QMHP discussed the negative attitude that had been observed on numerous occasions. I: QMHP explored client’s self-talk that mediates his frustration and anger (e.g., demanding expectations reflected in should, must, or have to statements); identify and challenge biases, assisting him/her in generating appraisals and self-talk that corrects for the biases and facilitates a more flexible and temperate response to frustration. QMHP and client engaged in a healthy physical outlet to further facilitate effective expression of feelings. QMHP assisted client to create another chart, receiving input from mother on continued expectations. QMHP provided client with additional stickers to utilize daily. R: Despite his continued difficulty communicating his feelings, and missing curfew 2 times client appeared proud of his self, for the progress made in regard to his chores and his additional positive behaviors (i.e. exercise, writing in his journal, acts of kindness, not asking his mother questions more than 2 times, etc.), as evidenced through his facial expressions and self-praise. R: Client was able to verbalize his understanding of the importance of regulating his emotions and expressing such through appropriate means. Client stated that he did not need to track behaviors anymore because he was good. Client expressed that he was ready to go back outside to chill with his friends, yet emphasized that he had enjoyed today’s session.
